Required Forms
Step 3
As an education provider, Haworth is required to use the following forms to meet compliance requirements for IIDA, AIA, and other organizations. Our courses are approved by two organizations: AIA and IDCEC. The IIDA, ASID, IDEC, IDC and NKBA are members of the Interior Design Continuing Education Council (IDCEC). Consequently, an IDCEC-approved course is automatically approved by all five organizations.
Sign-in Sheet
Personalize the form at each event and place on your registration table. Return the forms to Haworth.
Survey
Personalize the form and distribute it at registration. Close the event by reminding the audience to turn in the survey to receive a Certificate of Completion. Return surveys to Haworth.
Certificate
Personalize and make copies beforehand; the workshop facilitator or presenter signs the certificate. Distribute at the close of the event.
NEW: NCIDQ Online Submission Instructions
Effective July 1st 2011 NCIDQ submissions can only be done online. Please feel free to distribute this instructional half-sheet handout to participants during the first few months of the paper to online transition.
AIA Form “B”
Architects, to register AIA credit, must print his/her name and membership number on the form at your registration table. The form is returned to Haworth for processing.
